We do not collect personally identifiable information about you when you visit this site, unless you choose to give it to us by completing an online form or registering for our newsletter. If you do choose to give us your personal details (such as your name, email address, telephone number etc) they will be able to be accessed by our staff for the purposes for which you supplied it and/or so that we can better understand you and your interest in our services.

Why we collect information and how it is used

We collect personal information to help us perform our services and for other lawful purposes. For example, we may use or disclose personal information as reasonably required to communicate with you, obtain or verify information required in relation to your programme of treatment.

Information will be collected in accordance with the information privacy principles in the Privacy Act 2020.

Personal information about you collected and held by Merivale Physiotherapy Clinic is in paper-based or digital (electronic) format.  We may use cloud-based electronic storage technologies to store personal information. We will take reasonable efforts to protect the personal information we hold from loss, misuse, unauthorised access, disclosure, alteration, or destruction.  Please note there are some inherent risks associated with electronic technologies and the Internet are outside our control.

Can you access or request correction of your personal information?

You have the right to have access to any personal information we hold about you in such a way that it can readily be retrieved. You may also ask for it to be corrected if you think it is wrong, in accordance with the information privacy principles in the Privacy Act 2020.
